Please use the following information to fill out the flu shot registration (previously the “golden ticket”) form for Mid City Pediatrics’ flu vaccine drive-through/walk-in clinics.
Please note: You will need to fill out a form for each childoradult that plans to receive a flu shot at Mid City Pediatrics.
1. Log onto your child’s portal at midcity.pcc.com/portal.
- If you have not yet set up a portal account, please call our front desk at 318-221-2225 to assist with your account set up
- Each child and/or adult receiving a flu shot will need to have a profile on your patient portal. If their name does not show up when logged on, please call our office.
2. Click “Create Message”

3. Choose the name of the child/adult receiving the flu vaccine and then select “Flu Shot Registration”
Please note: You will need to fill out a form for each childoradult that plans to receive a flu shot at Mid City Pediatrics. If the name of the child/adult you are registering does not show up as a choice under “Patient,” please call our office to assist.

4. Fill out your child’s primary pediatrician, answer the questionnaire, and upload your insurance card and then press “Send”

5. Show up at one of the flu shot drive-through or walk-in dates to receive your flu shot!
- Drive Thru dates will be posted on Facebook/Instagram.
- No appointment needed — simply choose a day that works best for you and come in during the posted hours.
*️⃣Please note: It isVERY important to submit this information before coming for your shot. If it has not been submitted, we will not be able to give the flu shot, and you will need to return once the information is complete. *️⃣
